VitalCaring Group is seeking a Care Transition Coordinator to connect patients, families, and healthcare partners with hospice services. You’ll be the primary liaison for referrals and clinical teams, ensuring timely, seamless transitions of care across a field-based role.

Qualifications

• Bachelor’s degree in healthcare, business, marketing, or related field; or equivalent experience.
• Minimum of two years in healthcare or related sales experience.
• Strong communication, relationship-building, and influencing skills.
• Proficiency with Microsoft Office and CRM systems.

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams for seamless care transitions.
• Execute structured outreach and relationship-building calls to grow referral volume.
• Develop partnerships with hospitals and physician offices to support timely admissions.
• Track activities and document sales and referrals using CRM/EMR systems.
• Meet or exceed referral and admission goals.
