Job summary

Adoration Health is seeking a Care Transition Coordinator to facilitate patient transitions from healthcare facilities to home health or hospice care. The role requires evaluating eligibility, coordinating care plans, and arranging services such as DME and infusion in line with agency protocols.

The CTC acts as a liaison among the agency, referrals, and healthcare providers to ensure timely communication and patient education.

Qualifications

• Minimum of 1 year of experience in home health or hospital-based case management.
• Preferred: 1–3 years of experience in medical marketing or healthcare business development.
• Active licensure in RN, LPN, SW, or PT in the practicing state is required.
• Must possess a valid driver’s license and reliable transportation.
• Understanding of Medicare/insurance coverage guidelines is required.

Responsibilities

• Achieve monthly production goals and Medicare-certified (MC) admission targets for assigned locations.
• Coordinate transfer orders and arrange ancillary services (DME, infusion).
• Educate patients on home health or hospice orders and related services.
• Communicate with referrals and providers to ensure seamless transitions.
• Maintain documentation and coordinate readmission status with the case management team.
