Job summary

AdvantageCare Physicians (ACPNY) is seeking a Care Team Associate I to support non-clinical services and population health initiatives. The role works under the supervision of the Practice Administrator and in collaboration with the Nurse Manager to ensure efficient care delivery.

The position requires basic medical terminology understanding, strong customer service, and the ability to support the care team in delivering best-in-class patient care across ACPNY’s network.

Qualifications

• High School diploma or GED.
• Associate degree preferred.
• 3-5 years' experience in customer service/care coordination in an ambulatory care setting.

Responsibilities

• Provide general nursing care, administer medications, and coordinate care with the care team.
• Warmly greet patients and manage wait times, patient flow, and communication.
• Answer phones professionally; schedule appointments; verify insurance; resolve patient concerns.
• Check-in patients; register demographics and financial information; ensure data integrity; scan documents.
• Check-out patients; ensure required documentation; collect feedback.
• Take and relay messages; escalate calls as needed; manage emails and tasks.
• Collect cash; provide receipts; perform end-of-day reconciliations; manage scheduling reminders.
• Coordinate referrals and tests; arrange appointments; provide directions and records.
• Review referrals; liaise with external specialists; follow up as needed.
• Assist campaigns/outreach to at-risk patients to reduce ED visits.
• Maintain communication with patients and health-care team.
• Attend meetings; show initiative; may be reassigned as needed.
• Demonstrate proficiency with electronic health records and practice management systems.
• Arrive on time; protect patient confidentiality; be a team player.
• Performs other duties as required.
