Job summary

AdvantageCare Physicians (ACPNY) is seeking a Care Team Associate I to support the care team with non-clinical and administrative tasks focused on population health. This role operates under the supervision of the Practice Administrator and collaborates with the Nurse Manager to ensure smooth patient flow.

The ideal candidate has strong customer service, basic medical terminology, and the ability to coordinate scheduling, registrations, referrals, and follow-ups to deliver high-quality care

Qualifications

• 3-5 years of experience in customer service/care coordination in ambulatory care
• Experience and ability to navigate an EMR
• Basic medical terminology knowledge and strong customer service skills

Responsibilities

• Provide general nursing care to patients including administering prescribed medications and treatments per orders
• Greet patients, manage wait times, and communicate with patients and guests
• Answer phones professionally and schedule appointments in the computer system
• Register patient demographics and financial information and ensure data integrity
• Check-out patients, collect payments, and perform end-of-day reconciliations
• Coordinate internal and external referrals and ensure timely communication of referrals and test results
• Assist care teams with campaigns and outreach to identified at-risk patients
