Job summary

AdvantageCare Physicians (ACPNY) is seeking a Care Team Associate II to support the treatment and care of patients, assist providers with clinical services and perform administrative tasks under the supervision of the Practice Administrator and RN/LPN.

You will handle patient check-in/out, verify insurance, manage data in the EMR, coordinate referrals, and support campaigns like influenza outreach to reduce ED visits. Strong customer service and data accuracy are essential.

Qualifications

• High school diploma or GED required.
• Associate degree preferred in related field.
• Certificate of completion from an accredited Medical Assistant program or eligibility to sit for certification within 18 months.

Responsibilities

• Greet patients warmly and manage patient flow in the waiting area.
• Answer calls professionally and schedule appointments; verify insurance.
• Register patients and ensure accurate demographic/financial data.
• Check-out patients and collect payments; provide receipts.
• Coordinate referrals and tests with EPIC EMR; relay information to patients.
• Support care teams in campaigns and outreach to at-risk patients.
