Job summary

CNS Cares, LLC is seeking a Care Manager to schedule direct care staff for patient visits, maximize authorization units, and advocate for patient needs while ensuring timely, accurate service and high patient satisfaction.

You will coordinate with nursing and leadership, utilize EMR systems, and partner with the authorization team to ensure appropriate care levels. In-office, full-time role with Monday–Friday schedule.

Qualifications

• High School Diploma is required.
• Two (2) years of healthcare experience or an equivalent combination of education and EEOICPA experience.
• Two (2) years of staff scheduling experience required.
• Must be at least 21 years of age.
• Associate's or Bachelor's degree preferred.
• At least two (2) years of DoL/EEOICPA experience.
• Home health experience preferred.

Responsibilities

• Schedule visits for the care team within the EMR.
• Find coverage when staffing needs arise.
• Source and hire new staff with recruiting and leadership teams.
• Ensure all authorized hours are used and scheduled per guidelines.
• Coordinate with DME providers to meet patient needs.
• Advocate for appropriate care levels and renewals for patients.
• Assist authorization teams with start of cares and renewals.
• Attend medical appointments to support care coordination.
• Gather medical records as needed.
• Ensure staff are scheduled with appropriate nursing or home health aid coverage.
• Provide EMR and home orientation training to staff.
• Participate in an on-call rotation.
