Job summary

On behalf of CNS Cares, LLC, the Care Manager oversees the delivery of coordinated care for patients, advocating for needs and ensuring timely, high-quality service. This role focuses on maximizing authorization units, communicating with physicians, and ensuring patient satisfaction across home health settings.

The position requires strong leadership, adherence to HIPAA, and the ability to manage staffing schedules while coordinating with multiple teams in a fast-paced healthcare environment.

Qualifications

• High School Diploma and Associates Degree in a related field.
• At least two (2) years of healthcare experience or equivalent EEOICPA experience.
• At least two (2) years of staff scheduling experience.

Responsibilities

• Coordinate care delivery, maximize authorization units, and ensure patient satisfaction.
• Schedule visits in the EMR and arrange staffing coverage.
• Source and hire staff with local leadership to meet authorized hours.
• Coordinate DME needs with suppliers to meet patient requirements.
• Assist with start of care and renewal periods for appropriate care levels.
• Attend medical appointments with patients as a resource to clinicians.
• Ensure compliance with company and state guidelines.
• Provide EMR and home orientation training to staff.
• Participate in an on-call rotation.
• Adhere to all company policies.
