Job summary

Planstin, based in Southern Utah near Saint George, Utah, seeks a Care Coordinator to serve as a central contact for members’ healthcare needs and to build strong provider relationships.

You will coordinate appointments, referrals, and billing codes while helping members access Planstin and Primestin benefits. The role involves outreach across the region and collaboration with the claims team to ensure smooth care delivery.

Responsibilities

• Coordinate in-person and urgent care appointments.
• Arrange lab work, imaging, and other services.
• Assist members with navigating Planstin and Primestin benefits.
• Manage healthcare referrals and follow-up care.
• Help members access virtual care, specialties, labs, and providers.
• Collaborate to collect billing codes for surgeries and major procedures.
