Job summary

Planstin Administration, Inc. is seeking a Care Coordinator in Southern Utah to centralize member care and collaborate with providers.

You will coordinate appointments, referrals, labs, imaging, and virtual care while guiding members through Planstin and Primestin benefits to improve the patient experience. This role demands strong communication, organization, and empathy in a fast-paced environment, with a focus on coordinating care across multiple services and locations, including outreach

Qualifications

• Experience coordinating care across multiple services and providers.
• Excellent communication and interpersonal skills.
• Strong organizational skills and attention to detail.
• Ability to manage multiple tasks in a dynamic environment.
• Experience with healthcare billing and provider relations is a plus.
• Commitment to enhancing patient experience through coordinated care.

Responsibilities

• Coordinate in-person and urgent care appointments.
• Arrange labs, imaging, and other healthcare services.
• Assist members with navigating Planstin and Primestin benefits.
• Manage referrals and coordinate follow-up care.
• Help members access virtual care, specialists, labs, and other providers; including outside St. George as needed.
• Collaborate with members and providers to collect billing codes for surgeries and major procedures; liaise with the claims team.
