HopeSource in Wenatchee, WA is seeking a Care Navigator for the Housing team. This full-time role involves providing comprehensive support to clients, connecting them to HopeSource programs and vetted community resources, and helping address barriers to housing and essential services.

Responsibilities include offering a trauma-informed, person-centered approach, coordinating with internal and external partners, and advocating for clients to achieve long-term stability and independence.

Formación

• Ability to communicate with diverse clients in a respectful manner.
• Experience providing case management or social services is preferred.
• Familiarity with community resources and referrals.
• Strong organization and time-management skills.
• Ability to advocate for clients and navigate complex systems.

Responsabilidades

• Provide a single point of contact to address a wide variety of client needs.
• Connect clients to HopeSource programs and vetted community resources.
• Address barriers to housing, food, transportation, and utilities.
• Support workforce development, health care, and life skills education.
• Coordinate with partner agencies to ensure continuity of care.
• Advocate for clients and help them reach self-sufficiency.
• Navigate systems and remove barriers where possible.
• Engage in client-focused case conferencing with partners.
• Perform tasks with little supervision to meet program priorities.
