Job summary
Mount Rogers Community Services seeks a Housing Navigator to provide assessments, planning, and referral coordination for individuals in supportive housing settings. The role emphasizes person-centered plans, monitoring outcomes, and linkage to medical, behavioral, and community resources.

Qualifications
- Bachelor’s degree in a Human Services or related field preferred.
- Must meet QMHP-A or QMHP-A Trainee registration requirements.
- Valid driver’s license with safe driving record.
Responsibilities
- Provide housing service navigation and assess functional needs.
- Refer, advocate, and monitor service delivery for quality of care.
- Support community integration and daily living skills.
- Coordinate referrals to medical, behavioral, and social programs.
- Assist with discharge planning and housing reintegration.
- Document all services in the electronic health record.
- Attend supervision, staffings, and trainings to improve practice.
- Conduct monthly in-home visits to assess housing stability.
