Qualifications

• High school diploma or equivalent required; associate or bachelor's degree in human services, social work, psychology, criminal justice, behavioral health, or related field preferred.
• Experience in human services, behavioral health, substance use services, care coordination, case management, community outreach, or related field preferred.
• Knowledge of community resources and service systems related to behavioral health, substance use treatment, healthcare, housing, benefits, and other needs.
• Ability to establish positive relationships, maintain boundaries, and work with individuals facing multiple barriers.
• Strong organizational and follow-through skills to coordinate referrals, appointments, services, and documentation.

Responsibilities

• Maintain a flexible schedule including some community-based and evening work.
• Provide individualized, person-centered care navigation based on identified needs and goals.
• Engage individuals in identifying priorities and connecting with services and resources.
• Assist with understanding services, eligibility, and options for care and supports.
• Coordinate services with treatment providers, healthcare, housing, and community resources; document accurately.
