Job summary

City Care Inc. seeks a Housing Stability Advocate to support residents transitioning from homelessness with home visits and wrap-around services. You will help residents set goals within a psychosocial rehabilitation framework to achieve housing stability and retention.

You will collaborate with Housing Navigators, City Care programs, and community partners to coordinate referrals, documentation, and direct placement when eligible, promoting independence and well-being.

Qualifications

• Experience in case management, recovery services, behavioral health, or related fields.
• Ability to use a computer and accurately enter data; HMIS experience preferred.
• Pre-employment background check, driving record review, and drug screening required.

Responsibilities

• Carry out City Care’s mission, vision, and values.
• Build knowledge of community resources and relationships with service providers.
• Conduct home- and community-based visits to support housing stability and independence.
• Help residents set goals, develop service plans, and monitor progress over time.
• Support residents in health, recovery, finances, employment, education, relationships, and independence.
• Provide case management, resource coordination, crisis intervention, and advocacy.
