Responsibilities

• Conduct health-related social needs screenings and assessments.
• Identify barriers related to housing, transportation, food security, employment, healthcare access, behavioral health, and financial stability.
• Provide referrals and navigation support for healthcare, behavioral health, housing, utilities, food, transportation, workforce services, public benefits, and family resources.
• Assist participants with applications, eligibility determination, appointment scheduling, and service access.
• Conduct follow-up activities and maintain closed-loop referrals.
• Coordinate services among providers and community partners.
• Receive and process referrals from healthcare providers, schools, and community organizations.
• Maintain accurate participant documentation and referral tracking records.
• Conduct outreach activities, presentations, resource fairs, and community engagement efforts.
• Maintain current knowledge of community resources.
• Participate in training, supervision, and quality improvement activities.
