Job summary

One Senior Care seeks a compassionate Enrollment Specialist to guide seniors and families through joining our programs. You’ll conduct assessments, manage documentation, and coordinate steps with the Interdisciplinary Team to ensure a smooth enrollment experience.

You’ll work with participants, families, and community partners to achieve successful transitions. You will join a mission-driven team that values communication, teamwork, and participant support, with training and development

Responsibilities

• Meet with potential participants and families to explain the program and assess needs.
• Complete in-home and community-based assessments for prospective participants.
• Coordinate required evaluations, documentation, and intake steps.
• Attend intake appointments and support participants through enrollment.
• Complete intake and enrollment packets accurately and on time.
• Serve as main point of contact for referrals and guide them through enrollment stages.
• Present referrals to Interdisciplinary Team for review and collaboration.
• Maintain records and updates in CRM and EMR.
• Work with Medicaid Enrollment Specialist to support Medicaid processing.
• Participate in team meetings, training and quality improvement.
• Track weekly and monthly goals and enrollment progress.
• Identify barriers to enrollment and create solutions.
