Job summary

NextStep Care in Duluth, GA is seeking a Care Manager for Care Management Services to act as a liaison between members and home and community-based service providers. You will educate families, develop collaborative relationships with providers, and monitor member wellbeing.

The role requires a Bachelor’s degree in social work or related field with 2 years of experience, and experience in social work or geriatrics is preferred.

Qualifications

• Bachelor’s degree in social work or related human services field is required with 2 years of work experience.
• Experience in social work, home and community-based services, healthcare or geriatrics preferred.

Responsibilities

• Acts as a liaison between members and home and community-based service providers to facilitate quality and cost-effective care and outcomes.
• Provides education and support to members and their families regarding community resources and services.
• Develops effective, collaborative relationships with primary care providers, managed care plan providers, HCBS providers, caregivers and families.
• Maintains regular communication with members through phone calls and home visits to monitor wellbeing and adjust care plans as needed.
• Records and documents interventions and member information completely and accurately, per payer and organization guidelines.
• Collaborates with internal teams to ensure continuity of care for assigned members.
• Identifies and resolves care plan variances to ensure effectiveness of member’s care plan.
• Addresses member engagement and behavior to support optimal health and functioning.
• Participates in after-hours on-call rotation as assigned.
• Maintains quality of services through collaboration with QA and Education Team and Care Management Director.
