Qualifications

• Valid Georgia LPN license with 2 years of experience in health and human services.
• Experience in social work, home and community based services, healthcare or geriatrics preferred.
• Valid Driver’s License and reliable transportation.

Responsibilities

• Observe members in home environments gathering medical, environmental, and psychosocial data.
• Accept referrals and screen potential members in a timely manner.
• Address member or caregiver concerns effectively and efficiently.
• Assist in carepath developments and HCBS services.
• Record and document member information accurately per guidelines.
• Collaborate with the multidisciplinary team to address primary care needs.
