NextStep Care in Georgia is seeking a compassionate Social Service Case Manager to join our mission-driven team. You will work closely with members, families, and care providers to coordinate quality, cost-effective home and community-based services.

This role emphasizes communication, advocacy, and organization as you monitor wellbeing, document interventions, and support care plans. You will collaborate with internal teams and participate in after-hours on-call rotations.

Qualifications

• Bachelor’s degree in social work or related field.
• At least 2 years of social work, home-based services, healthcare or geriatrics preferred.

Responsibilities

• Liaison between members and home/community service providers to ensure quality and cost-effective care.
• Educate and support members and families about community resources and services.
• Build collaborative relationships with primary care and HCBS providers and families.
• Maintain regular communication through calls and home visits to monitor wellbeing and adjust care plans.
• Document interventions accurately per payer and organization guidelines.
• Support care management team to ensure continuity of care.
