Job summary

Qualifications

• Associates degree, Diploma, or Bachelor's in Nursing (preferred).
• RN license is required (Single State or Compact).
• Minimum 1 year nursing experience in acute care.
• Excellent written and verbal communication skills.
• Ability to work under pressure in a fast-paced environment and collaborate with care teams.

Responsibilities

• Assess transitional care needs and coordinate across the care continuum.
• Engage with patient and family to ensure discharge planning goals are met.
• Participate in interdisciplinary rounds and communicate with care teams.
• Document care progression and all discharge planning steps clearly.
• Collaborate with physicians, social workers, and payers to resolve barriers to discharge.
