Job summary

Wellstar Health System in Georgia is seeking a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate across the continuum, and engage with patients and families to ensure discharge plans meet goals.

The CC RN will partner with PAS, financial counselors, and UM to align coverage and resources, while actively participating in rounds and care conferences to promote timely, efficient care progression.

Qualifications

• Associates or Diploma in Nursing, BSN preferred
• RN licensure (single state or compact)
• BLS certification or equivalent
• 1 year nursing experience in acute care
• Excellent communication, organization and problem-solving skills

Responsibilities

• Assess transitional care needs and coordinate care across the continuum.
• Engage with patient and family to ensure needs are met and discharge goals are clear.
• Collaborate with physicians, care team, and UM to support plans and resources.
• Manage discharge planning and track progression of care.
• Participate in interdisciplinary rounds and care conferences.
