Job summary

Wellstar Health System in Georgia seeks a Care Coordinator RN (CC RN) to assess transitional care needs and coordinate across the continuum. You will engage with patients and families to ensure discharge planning is effective and care goals are met.

You will collaborate with physicians, the utilization review nurse, and care team to manage timelines, align resources, and document progress in the electronic medical record, contributing to smooth transitions and quality care.

Qualifications

• Associates or diploma nursing degree required.
• RN licensure in state (or Compact) per Georgia.
• BLS certification required.

Responsibilities

• Assess patients' chronic disease management needs and psychosocial risk factors.
• Coordinate discharge planning and post-acute referrals.
• Collaborate with physicians, UM nurse, PAS and care team to support care progression and discharge plans.
• Document initial assessments and ongoing progress in the EMR.
• Assist patients and families with education and transition to next level of care.
