Job summary

Wellstar Medical Group, LLC is seeking a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate across the continuum, and engage patients and families to ensure care needs are met. You will manage discharge planning, collaborate with physicians, and optimize resource use while supporting patient education and care progression.

The role requires RN licensure in Georgia (or compact), BLS certification, and at least one year of acute care nursing experience.

Qualifications

• Associates degree in nursing or nursing diploma; BSN preferred.
• RN license in Georgia or compact (multi-state).
• BLS certification required.
• Minimum 1 year nursing experience in acute care.

Responsibilities

• Assess patients' chronic disease management needs and psychosocial risk factors.
• Coordinate discharge planning with care team and family.
• Collaborate with physicians and care team to ensure timely progression and discharge.
• Document interactions and care plans in medical records; track avoidable days.
