Job summary

Wellstar Health System is seeking a Care Coordinator RN (CC RN) in Georgia to assess transitional care needs, coordinate care across the continuum, and engage with patients and families to ensure care needs are met.

The CC RN will plan effectively to meet patient needs, manage length of stay, and promote efficient resource use. Requires RN license, BLS, and at least 1 year of acute care nursing experience.

Qualifications

• Associates Nursing or Diploma Nursing or Bachelors Nursing-Preferred.
• RN license (State/Compact) and BLS certification.
• Minimum 1 year nursing experience in acute care setting.

Responsibilities

• Assess transitional care needs and psychosocial risk factors.
• Coordinate discharge planning and care progression with physicians and care team.
• Collaborate with PAS, financial counselor and UM nurse to review insurance requirements.
• Conduct patient/family education and participate in interdisciplinary rounds.
• Document assessments and care plans in the medical record.
