Job summary

Arkansas Provider Coalition, LLC is seeking a Care Coordinator I to manage member care plans, coordinate with families and providers, and ensure timely documentation in the EMR system. • One year of beginner-level experience with Microsoft Excel, Outlook, and Word.
• Bachelor's degree in social work, psychology, sociology, or health-related field; or two years of college coursework in those areas and one year of relevant experience.

Responsibilities

• Develops and updates the annual Person-Centered Service Plan (PCSP) for assigned members.
• Maintains monthly contact with members, families/guardians, providers, and other care team members.
• Documents activities in EMR within 24 hours and adheres to APC policies.
