Job summary

Arkansas Provider Coalition is seeking a Care Coordinator I to join the Care Coordination department. The role requires conducting PCSP development, maintaining regular contact with members and families, and coordinating with care teams. The position is full-time and safety-sensitive, with travel and driving expected.

Applicants must meet minimum education/experience requirements and operate in compliance with HIPAA and Medicaid standards; a reliable home office and transportation are essential.

Qualifications

• One year of beginner-level experience with Microsoft Excel, Outlook, and Word.
• Ability to communicate effectively in English and/or Spanish.

Responsibilities

• Develops, documents, evaluates, and updates the annual Person-Centered Service Plan (PCSP) for each member.
• Maintains monthly contact with members, families, providers, and care team members.
• Conducts in-person visits quarterly; more for complex cases; documents activities in EMR within 24 hours.
• Maintains compliance with HIPAA and Medicaid regulations.
