Job summary

Arkansas Provider Coalition, LLC is seeking an Enhanced Care Coordinator to develop and monitor person-centered service plans for complex cases across Arkansas. Remote/field-based role with in-state residency required, ensuring timely access to quality care.

You will work with members, families, and providers, conducting weekly care team calls and coordinating appointments while maintaining strict data confidentiality and compliance.

Qualifications

• Minimum 2 years of college coursework in social sciences or health-related field.
• Bachelor’s degree in social work, psychology, sociology or related field preferred.
• Experience in care coordination and familiarity with managed care a plus.

Responsibilities

• Work directly with members and support circles to meet health and service needs.
• Regularly communicate with members, families, providers, and stakeholders.
• Assist with unassigned cases to ensure smooth care transitions.
• Conduct weekly calls with care teams to coordinate care.
• Schedule and manage appointments and travel across Arkansas.
