Job summary

Arkansas Provider Coalition is seeking a Care Coordinator I to support Summit Community Care by coordinating services for individuals with developmental disabilities and behavioral health needs. The role focuses on patient-centered care, referrals, follow-ups, and navigation to promote independence.

The position requires a valid Arkansas driver’s license, reliable vehicle, and willingness to travel to member homes and provider offices. Comprehensive benefits are provided.

Qualifications

• Bachelor’s degree in social work, psychology, sociology, or related health field (or 2 years college + 1 year experience).
• Proficiency with Microsoft Office (Excel, Outlook, Word).
• Knowledge of community resources to integrate services into member care plans.
• Proficiency with electronic filing systems and record-keeping.
• Ability to use iPhone and apps for communication and record management.

Responsibilities

• Develop, document, evaluate, monitor, and modify members’ annual PCSP.
• Ensure members’ health and service needs by coordinating with members, families/guardians, providers, and support circle.
• Maintain monthly contact with members, families, providers, and community resources.
