Job summary

Arkansas Provider Coalition is hiring a Care Coordinator I to develop and monitor PCSPs for members in Pulaski County, AR. The role requires field visits, monthly contacts, and in-person visits quarterly, with documentation in the EMR within 24 hours.

The position emphasizes HIPAA compliance, coordination with families, providers, and the care team, and adherence to APC policies while supporting members’ health and safety needs.

Qualifications

• One year of beginner-level experience with Excel, Outlook, and Word.
• Bachelor's degree in social work, psychology, sociology, another social science, or a health-related field.
• Equivalent combinations of education and experience may also be considered.

Responsibilities

• Develops, documents, evaluates, monitors, and modifies the Person-Centered Service Plan (PCSP) for assigned members.
• Maintains monthly contact with members, families/guardians, providers, and the care team.
• Performs quarterly in-person visits and more frequent monthly visits for complex cases.
• Ensures compliance with HIPAA, Medicaid guidelines, and APC policies.
