Responsibilities

• Outreach to members via phone or home visits to discuss care plan and next steps.
• Coordinate care activities with providers, partners, and members.
• Respond to inquiries about care plan and services.
• Communicate with care managers and practitioners to facilitate services.
• Support service assessments and document member needs.
• Maintain member records per state/regulatory requirements.
• Follow policies and standards.
• Identify needs and refer to Care Manager or community resources.
• Educate members on benefits and available resources.
• Perform other duties as assigned.
• Complies with all policies and standards.
