Job summary

ARPCO Arkansas Provider Coalition, LLC is seeking a Care Coordinator I in Lonoke County, AR. The role focuses on developing and updating each member’s Person-Centered Service Plan (PCSP), coordinating with families, guardians, and care teams, and ensuring timely documentation and HIPAA compliance.

Responsibilities include quarterly in-person visits, monthly check-ins, and using EMR systems to track progress and outcomes.

Qualifications

• One year of beginner-level experience with Microsoft Excel, Outlook, and Word, or equivalent
• Bachelor’s degree in social work, psychology, sociology, or a health-related field required
• Equivalent combinations of education and experience will be considered

Responsibilities

• Develops, documents, evaluates, monitors, and modifies the member’s annual PCSP on an ongoing basis
• Maintains monthly contact with members, families, providers, and care team
• Conducts in-person visits at least quarterly with monthly visits for complex cases
• Documents in EMR within 24 hours per APC policies
• Maintains confidentiality and HIPAA compliance
