Arkansas Provider Coalition is seeking a Care Coordinator I to support care coordination activities across Summit Community Care. This remote/mobile role involves developing and updating Person-Centered Service Plans, conducting visits, and coordinating with families, guardians, providers, and the care team to ensure timely, high-quality services.

Qualifications

• One year of beginner-level experience with Microsoft Excel, Outlook, and Word, plus a bachelor’s degree in social work, psychology, sociology, another social science, or a health-related field.
• One year of beginner-level experience with Microsoft Excel, Outlook, and Word, plus two years of college coursework in Social Work, Psychology, Sociology, another social science, or a health-related field, and one year of experience working with individuals with developmental, intellectual, and/or behavioral health disabilities, and/or substance use disorders.

Responsibilities

• Develops, documents, evaluates, monitors, and modifies the assigned members’ annual PCSP on an ongoing basis, including member choices across Behavioral Health, Community Resources, Crisis Safety, IDD/Waiver, Financial Information, Mental Health, Physical Health, and Substance Abuse.
• Maintains monthly contact with members, their families/guardians, providers, community resources, stakeholders, and other professionals in the members care team.
• Conducts in-person visits at least once every quarter, with more frequent monthly visits for complex cases.
• Documents work activities and member information into the EMR within twenty-four hours according to APC policies.
• Maintains compliance with HIPAA and Medicaid regulatory standards.
