Job summary

Quality Care LLC is seeking a Care Coordinator / Direct Service Professional in Minnesota. This hybrid role combines administrative support with in-person care delivery, spending part of your time with the Care Coordination team and the remainder providing direct support to clients in their homes.

You will complete paperwork, support scheduling, and assist with daily living activities and client well-being, while developing independent living skills in the home environment.

Qualifications

• High school diploma or equivalent required.
• Strong organizational and communication skills.
• Experience with office software and ability to handle paperwork accurately.
• Compassionate, patient, reliable, and detail-oriented personality.

Responsibilities

• Complete paperwork and documentation related to client services, including data entry and filing.
• Support scheduling, communications, and office organization to ensure smooth daily operations.
• Provide companionship, emotional support, and assistance with daily living activities such as meal preparation, housekeeping, and transportation.
• Help clients develop and maintain independent living skills in their home environment.
• Collaborate with other Care Coordinators and staff to promote client well-being.
