Job summary

CareAbout in Florham Park, NJ is seeking a CMA-CC to support patient continuity of care within a multidisciplinary primary care team. You will coordinate with patients, families, physicians and nurses to identify needs, reinforce discharge plans, arrange follow-ups, and address barriers to care.

Qualifications

• Certified or Registered Medical Assistant.
• At least 2 years of similar job experience in a medical office preferred.
• Familiar with EKG, Phlebotomy and Patient Care.

Responsibilities

• Monitor and identify patients eligible for Transitional Care Management after hospital or post-acute discharge.
• Complete interactive contact with patient/caregiver within TCM timeframe.
• Establish contact with patients by phone or approved methods.
• Review discharge instructions and identify questions or barriers.
• Confirm understanding of discharge plan and follow-up recommendations.
• Identify medication-related concerns and communicate to the care team.
• Arrange timely follow-up appointments with the primary care provider.
• Coordinate with providers, nurses, care managers, and others to support continuity of care.
• Escalate clinical symptoms to licensed staff as per protocols.
• Document all patient contacts and interventions in the EHR/EMR.
• Support care team to reduce avoidable ER visits and readmissions.
• Participate in care team meetings and care coordination activities.
• Maintain HIPAA compliance and confidentiality.
