Job summary

Frederick Health is seeking a Care Management Assistant (CMA) to support the Integrated Care Management team. The CMA conducts outreach, assesses patients during transitions, and closes care gaps related to quality measures, while performing a range of clinical and administrative tasks under supervision.

The role collaborates with various Frederick Health departments and community partners, following compliance programs and behavior standards.

Qualifications

• AA degree with 3–5 years of clinical work experience preferred.
• Certified Medical Assistant or Certified Nursing Assistant certification preferred.
• Must have current CPR certification.

Responsibilities

• Outreach to patients during transitions to assess understanding and close care gaps.
• Generate orders/referrals, obtain medical records, and schedule appropriate appointments.
• Collaborate with primary care team and community partners.
• Access and utilize multiple software systems to identify target patients and document.
• Respond to calls and messages from patients and care team.
