Job summary

Frederick Health Hospital, Inc. is seeking a Care Management Assistant (CMA) to support the Integrated Care Management team by conducting outreach and assessing patients during transitions in care and by closing care gaps related to quality measures.

The CMA will collaborate with the primary care team and community partners, access multiple software systems, and perform a wide range of clinical and administrative tasks as directed by the care management staff.

Qualifications

• AA degree with 3–5 years of clinical work experience in a healthcare setting preferred.
• Certified Medical Assistant or Certified Nursing Assistant certification preferred.
• Must have current CPR certification.

Responsibilities

• Outreach to patients during care transitions to assess understanding and follow up on discharge instructions.
• Close quality measure gaps by coordinating orders/referrals and education on health maintenance.
• Access multiple software systems for target patient identification and documentation.
• Collaborate with primary care teams, care management staff, and community partners.
• Schedule appointments and respond promptly to patient and care team messages.
