Integrated Services for Behavioral Health (ISBH) in Ohio seeks a Care Coordination Supervisor to lead the care coordination team and ensure adherence to the high-fidelity wraparound model.

Salary ranges from $52,609.67 to $63,282.33 based on experience, with a $5,000 hiring bonus; benefits include medical, dental, vision, 401K with employer match, and EAP. ISBH serves Southeast and Central Ohio with community-focused services.

Qualifications

• Bachelor’s or Master’s degree in social work, education, counseling, psychology preferred.
• Three years of experience in care coordination.
• Previous supervisory experience preferred.
• Experience in behavioral health with multi-system youth or autism appreciated.
• Knowledge of behavioral health regulations and disabilities services.

Responsibilities

• Supervise care coordination team via meetings and regular communications.
• Provide training for direct care coordinators in home and community settings.
• Oversee documentation and care plans to meet Medicaid standards; review and approve reports.
• Manage on-call rotation and fidelity review activities.
• Collaborate with systems and partners to ensure family voice and quality services.
