Qualifications

• LSW/LPC preferred.
• Three years of experience in care coordination.
• Previous supervisory experience preferred.
• Behavioral health experience required.
• Knowledge of wraparound services and Medicaid standards.

Responsibilities

• Supervise care coordination team via meetings and communications.
• Provide training for direct care coordinators in home and community.
• Deliver routine staff training and skill development.
• Promote teamwork and recognition culture.
• Ensure completion of high fidelity wraparound training credentials.
• Coordinate on-call rotation and fidelity monitoring.
