Job summary

CVS Health Corporation is seeking a Case Management Coordinator for a full-time telework role with significant travel in and around Northwest Illinois. You will evaluate members, coordinate care plans, and collaborate with clinical teams to ensure optimal outcomes.

The role requires 2+ years in Behavioral Health, with strong documentation skills and the ability to travel 50-75%. A Bachelor’s or Master’s in Behavioral Health or Human Services is preferred.

Qualifications

• 2+ years of experience in Behavioral Health or related field.
• Willing and able to travel 50-75% of the time in Winnebago, Boone, Stephenson, Ogle and Jo Daviess Counties, IL.
• 2+ years of experience with electronic documentation and Microsoft Office applications.

Responsibilities

• Evaluate members’ needs and eligibility and recommend an approach to case resolution.
• Identify high risk factors and refer to clinical case management or crisis intervention as needed.
• Coordinate and monitor care plan activities and progress.
• Consult with medical directors and health programs to overcome barriers to goals.
• Escalate quality of care issues through established channels.
• Negotiate options and services to meet members’ benefits and healthcare needs.
• Engage members to promote healthy lifestyle changes and independence in decisions.
