Qualifications

• 2+ years experience in behavioral health, social services or related field.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Conduct comprehensive evaluation of member's needs/eligibility and recommend a case resolution approach based on benefit plan and available programs.
• Identify high risk factors and coordinate care plan progress.
• Collaborate with case managers, supervisors, Medical Directors and health programs to overcome barriers to goals and ensure optimal outcomes.
• Coordinate and monitor care plan activities and ensure documentation in required systems.
