Job summary

Infinite Health Collaborative (i-Health) is seeking a Care Coordinator Float to support clinic and physician teams with administrative and patient care tasks. The role is full-time, Mon-Fri, 8:00 AM - 4:30 PM, across multiple Minnesota locations including Plymouth, Maple Grove, Robbinsdale, Brooklyn Park, and Blaine.

You will act as a liaison between providers and patients, manage physician schedules, arrange appointments and tests, and handle referrals per insurance guidelines.

Qualifications

• High school diploma or GED required; administrative/medical office experience preferred.
• Knowledge of medical terminology is strongly preferred.
• Transcription experience is a plus.

Responsibilities

• Coordinate communication between providers and patients.
• Build and maintain physician scheduling templates.
• Schedule patient appointments, surgeries, and tests.
• Obtain required referrals/authorizations per insurance guidelines.
• Provide follow-up to patient inquiries and coordinate with providers.
• Process mail and correspondence and manage records transmission.
• Assist in clinic workflows, including rooming and basic support.
