The Care Coordinator Float will support the clinic and physician care team by providing administrative and patient care services. This is a full-time role in Minnesota with hours 8:00 AM - 4:30 PM, working at multiple locations including Plymouth, Maple Grove, Robbinsdale, Brooklyn Park, and Blaine.

Responsibilities include scheduling, referrals, patient follow-up, records management, and maintaining physician schedules; some transcription or scribing tasks may be required.

Qualifications

• Administrative or medical office experience is preferred.
• Knowledge of medical terminology is preferred.
• Customer service experience is required or preferred.

Responsabilités

• Provide communication and liaison between providers and patients.
• Build physician schedule and maintain scheduling template.
• Schedule patient appointments, surgeries, and diagnostic tests.
• Obtains referral and/or authorization requirements per insurance guidelines.
• Provide follow up to patient inquiries, contacting provider as necessary.
• Coordinate the transmission and acquisition of patient records.
• Maintain Physician schedule, communicating changes appropriately.
• Transcription or medical scribing may be performed as needed.
• Assist in clinic with rooming and other duties as assigned.
