Enhabit Home Health & Hospice is seeking a licensed practical nurse to coordinate patient care by assigning patients to field clinicians and ensuring seamless communication among patients, staff, physicians and caregivers.

Responsibilities include managing the on-call notebook, hospitalization logs and ensuring timely, coordinated care. Ideal candidates have a valid LPN license, CPR, and significant home health experience.

Qualifications

• High School Diploma or equivalent.
• Licensed Practical Nurse or Vocational Nurse license.
• Valid clinical license to practice.
