Job summary

Enhabit Home Health & Hospice in Augusta, GA is seeking a dedicated nurse scheduling coordinator to optimize patient visits and care coordination. You will work closely with field clinicians, patients, physicians and caregivers to ensure timely, quality home health service.

The role requires strong communication, organizational skills and current RN/LPN license with CPR. In-office attendance is expected to support teamwork, scheduling and patient outcomes across our growing home health network.

Qualifications

• Must have a High School Diploma or equivalent.
• Licensed practical or vocational nurse with current license.
• Valid state driver's license and automobile liability insurance.
• CPR certification; home health scheduling experience preferred.

Responsibilities

• Schedule patients to field clinicians and coordinate visits.
• Communicate with field staff, patients, physicians, referrals and caregivers to ensure continuous care.
• Maintain on-call notebook and hospitalization logs to support effective communication.
