Job summary

Enhabit Home Health & Hospice is seeking a licensed practical or vocational nurse to coordinate patient scheduling with field clinicians in Illinois. The role emphasizes communication with patients, physicians, and caregivers to ensure continuity of care and smooth scheduling workflows.

The ideal candidate possesses an LPN/LVN license, CPR certification, and at least one year of healthcare experience, with preference for Homecare Homebase (HCHB) experience and bilingual English/Spanish

Qualifications

High School Diploma or equivalent.
LPN/LVN license required.
Valid clinical license per state requirements.
At least one year of clinical healthcare experience.
CPR certification required.
Minimum one year of home health, hospice, or pediatric experience in last 24 months with staffing and scheduling knowledge.
Valid state driver license and automobile liability insurance.
Basic technology skills including operation of a mobile device.
Preferred Homecare Homebase (HCHB) experience.
Preferred bilingual English/Spanish communication.
In-office attendance required; remote work not allowed.
Must be able to drive in all weather conditions.

Responsibilities

Schedule patients to field clinicians.
Coordinate care with patients, physicians, referral sources, caregivers and service providers.
Maintain on-call notebook and hospitalization logs to enhance communication.
