Qualifications

• Must have High School Diploma or equivalent.
• Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license required.
• Minimum of one year of clinical experience in healthcare setting is preferred.
• Valid state driver license and automobile liability insurance are required.
• Good technology skills including operation of mobile devices and Microsoft Office.

Responsibilities

• Schedule patients to field clinicians and coordinate with staff, patients, physicians, and referral sources to ensure continuity of care.
• Maintain on-call notebook and hospitalization logs to improve communication among stakeholders.
