Job summary

Enhabit Home Health & Hospice is seeking an experienced nurse coordinator to schedule patients with field clinicians and ensure seamless care coordination and communication among patients, families and providers.

The role requires RN/LPN licensing, a valid driver's license, CPR certification, and preferred Homecare Homebase experience; in-office attendance is essential to support supervision, teamwork, and timely care delivery.

Qualifications

• High School Diploma or equivalent is required.
• Licensed practical or vocational nursing license is required.
• Valid clinical license in state requirements.
• Minimum of one year of clinical experience.
• CPR certification required.
• Recent home health, hospice, or pediatric experience within last 24 months.
• Valid state driver license and automobile liability insurance required.
• Basic technology skills including mobile devices.

Responsibilities

• Schedule patients to field clinicians.
• Communicate with field staff, patients, physicians, referrals, caregivers and other providers to maintain care coordination and continuity.
• Manage on-call notebook and hospitalization logs to enhance stakeholder communication.

Education

High School Diploma or equivalent

Licensed practical or vocational nurse (LPN/LVN)

CPR certification

Home health, hospice, or pediatric experience within last 24 months

Valid driver license

Auto liability insurance
