Enhabit Home Health & Hospice is seeking a Scheduling Coordinator to organize patient visits with field clinicians and maintain seamless care coordination. You will communicate with patients, staff, and physicians to ensure timely services and accurate documentation.

This role requires a nursing license, clinical experience, and strong technology skills; bilingual abilities and reliable transportation are a plus.

Competenze

• High School Diploma or equivalent required.
• LPN/LVN license required.
• Valid state license to practice and CPR certification required.
• At least 1 year of clinical healthcare experience.
• At least 1 year of home health, hospice, or pediatric experience within the last 24 months with staffing/scheduling knowledge.
• Valid driver's license and automobile liability insurance required.
• Basic demonstrated technology skills, including operation of a mobile device.
• Preferred bilingual English/Spanish communication abilities.
• Dependable transportation in good condition; ability to drive in various weather.

Mansioni

• Schedule patients to field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and service providers to coordinate care.
• Manage on-call notebook and hospitalization logs to improve communication among stakeholders.
