Job summary

Qualifications

• Bachelor's degree in health care, public health, or related clinical field.

Responsibilities

• Engages and motivates members in case management programs and supports family-centered care.
• Collects information for tailored assessments and refers when criteria for referral and licensed clinical intervention are met.
• Provides navigation support to access social determinants of health and coordinates transitions of care.
• Outreach to PCPs, specialists, and home care providers to facilitate referrals and educate families for self-management.
• Answers calls, schedules appointments, and assists members to resolve immediate needs; maintains records and case files.
