Job summary

Nationwide Children's Hospital is seeking a Case Management Extender to support navigation and access to community resources, engaging with patients and families across inpatient, outpatient, and home settings. This role helps coordinate care and builds relationships with local agencies and schools.

Responsibilities include supporting population health programs, documenting care, and providing education to enhance self-management.

Qualifications

Bachelor's degree with background in health care, public health, or related clinical field preferred.
Active BLS certification required.
2 years of experience in healthcare in a patient-facing role preferred.

Responsibilities

Engages and motivates members to participate in case management programs and clearly articulates goals and interventions.
Assists with development of family-centered care and referrals to resources.
Collects information for tailored assessments and refers onward when criteria for referral are met.
Facilitates communication among healthcare team and provides navigation support to members.
Connects families to resources addressing social determinants of health and accommodates diverse needs.
Manages transitions between settings and coordinates follow-up across the continuum of care.
