Qualifications

• Bachelor's degree in health care, public health, or a related clinical field.
• Active BLS certification, required.
• Valid driver's license and auto insurance as required by hospital policy.

Responsibilities

• Engages and motivates members to participate in case management programs.
• Collects information for tailored assessments and refers when criteria for referral are met.
• Provides navigation support to help families access resources and coordinate care.
• Develops patients' and families' self-management skills through education and resource provision.
• Prepares and maintains records and case files with progress and changes.
